Cable Modem User Agreement

As part of the subscription to Paragould Light Water and Cable's (PLWC) Cable Internet service, Customer is provided the above numbered cable modem. This modem is the property of Paragould Light Water and Cable, and must be returned to the offices at 1901 Jones Road, Paragould, AR 72450 in proper working order upon termination of service by either party. If the above numbered modem is not returned upon termination of services, the above named subscriber will be charged the replacement cost of the modem plus shipping and handling fees.

Paragould Light Water and Cable agrees to maintain the modem in proper operating condition and will replace the modem if it fails under normal operational circumstances, i.e. day-to-day use.

The Customer will be responsible for the replacement cost of the modem plus shipping and handling fees if the modem is stolen or physically damaged to the point of failure while assigned to the customer's account.

All other restrictions applying to the proper use of the Internet, listed on the Grnco.net Internet Service Agreement, also apply.
